CCMAP is a licensed, non-profit community program that assists the uninsured and underinsured patient with obtaining medications for their chronic disease states. This is accomplished completely free of charge to the patient through donations and grant funding. In cooperation with the patient’s provider, CCMAP gives each patient increased access to medications as well as patient education needed to manage chronic disease states such as diabetes, hypertension, high cholesterol, mental health, asthma, and COPD. Our program is dedicated to improving the quality of life for eligible residents of Cumberland, Harnett, Hoke, and Sampson counties.
